IMPORTANT INFORMATION FOR PARENTS . . . . .

The Hartland/Lakeside School District recognizes that health and safety issues affect a child's ability to learn. The following information will help acquaint you with the school health services provided by the district school nurse.

EMERGENCY HEALTH INFORMATION

It is absolutely vital that the school have a record of parents' home, cell, work and emergency phone numbers. Parents should inform the school of any emergency health information such as allergies, chronic health concerns, preferred hospital, family doctor and family dentist. This information is kept confidential and needs to be updated annually.

ABSENCES FROM SCHOOL

Parents need to call the school health room at 262-369-6725 to report an absence. The school nurse is able to track disease trends if parents communicate briefly what illnesses or symptoms your child is experiencing. Cases of strep throat, chicken pox, fifth's disease and pink eye must be reported to the school office. Excessive absences will require a doctor's excuse.

ILLNESS AND INJURY

Each building has a health room staffed by school personnel and volunteers trained in the care of sick and injured children, supervised by the district school nurse. Minor injuries and illness are treated by the staff and students are returned to class. Parents will be notified if it is necessary for children to be sent home and will be expected to make the necessary arrangements. In the event of a serious illness or injury, emergency medical services will be utilized and parents notified immediately.

MEDICATIONS

Parents and physicians are encouraged to have daily doses of medications administered at home whenever possible. When it is necessary for prescription medications to be administered at school, written permission forms from parents and their physician are required. Any over-the-counter medication must also come with a permission form and be in their original container. VISION AND HEARING SCREENING

The district nurse provides vision and hearing screenings to selected grade levels. These screenings are not intended to replace regular examinations by licensed health care professionals.

IMMUNIZATION RECORDS

All students must provide a written immunization record at entrance to school verifying the student has received the minimum immunizations required by law, or parents must file a waiver with the school district. Immunization records are audited annually and reports filed with the health department. Please be aware that the immunization requirements have changed for the 2009-2010 school year. The Waukesha County Public Health Department is offering several upcoming clinics to help you meet the updated immunization requirements.

SPECIAL HEALTH CONCERNS

The District Nurse is interested in providing the best possible care for your children while they are in school. The nurse is available for consultation regarding hospitalizations, surgeries, prolonged illnesses, or other health related problems of the student.The nurse is also available to work with parents and teachers in developing individual health plans for students with temporary or chronic health concerns. Summer is a great time to visit your child's physician to update forms and vaccinations. Below are some Action Plan forms that you and your physician can use to create the best emergency plan for your child.

Hartland-Lakeside School District Guidelines for Life-Threatening Allergies

The goal of the Hartland-Lakeside School District is to provde a safe environment for all children, including those with life-threatening allergies. Although the risk to students with these allergies in the schools cannot be completely eliminated, it can be greatly reduced.

The Hartland-Lakeside School District has developed and implemented guidelines for the care of students with life-threatening allergies. If your child has a severe or life-threatening allergy, including food allergies, please review the guidelines and complete the following forms, available for download below:

  • Annual School Emergency Form (salmon card)
  • Allergy Action Plan
  • Authorization to Administer Prescription Medication Form (if applicable)
  • Elementary Lunchroom Table Preference Form

When these forms are returned to the school health office, the district nurse will be in contact with you to review the emergency action plan specific to your child.
